High School Re-Registration Forms

All 8th Grade Students Must be Re-registered in the Lynbrook School District.

Student Schedules will not be mailed until all required forms and records are handed in to the Guidance Office at North Middle School.

• Central Registry Verification Form (ALL Must Submit)
• Affidavit for Homeowner (Only Homeowners must submit)
• Affidavit for Landlord (Only Renters must submit)
• Affidavit for Renter/Non-Owner (Only Renters must submit)
